What is IOP?
Intensive outpatient programming (IOP) is a structured, evidence-based level of behavioral healthcare designed for people who need more support than traditional weekly therapy, but don’t require 24/7 residential or inpatient care.
At Charlie Health, our virtual IOP provides personalized treatment for people struggling with substance use. By combining group, individual, and family sessions each week, plus medication-assisted treatment (MAT), our program helps clients build the tools they need for lasting recovery—while continuing to live at home and maintain their daily routines.
Whether someone is transitioning from detox or residential care or just starting their recovery journey, Charlie Health’s IOP meets them where they are. We integrate mental health and substance use treatment, offering both harm reduction and abstinence-based options to support each client’s unique goals.
With flexible scheduling, structured accountability, and data-driven treatment planning, our virtual IOP for substance use disorders makes high-quality care more accessible—and more effective—than ever before.
Curated groups
By bringing people with similar mental health and substance use challenges together, we’re able to create group environments where clients can learn how to build connections and foster long-term healing.
Individual therapy
One-on-one connections are critical to the IOP model for substance use disorders, which is why each client’s unique treatment plan includes a primary therapist for weekly individual sessions.
Family therapy
A leading predictor of success in IOPs is family involvement. Our virtual Intensive Outpatient Program’s family therapy component teaches communication and coping skills to create a more supportive home environment—both during and after treatment.

FAQs about Substance Use Disorder
See All FAQsWhat is virtual IOP?
Virtual Intensive Outpatient Programming (IOP) is a level of mental healthcare that is typically more structured and intensive than traditional once-weekly therapy. Treatment is delivered to clients directly at home.
There are two main reasons someone may enroll in an IOP:
- A recent discharge from a residential treatment program (behavioral health or substance use), partial hospitalization program, or an emergency department.
- If they are already attending weekly therapy but are continuing to struggle with their mental health.
IOPs include multiple group, individual, and family sessions per week. The goal of IOP is to help people manage their health and achieve lasting recovery while still allowing them to maintain their daily routines and responsibilities.
How does virtual IOP work at Charlie Health?
Our virtual Intensive Outpatient Program (IOP) at Charlie Health is a personalized treatment solution for mental health and substance use disorder. Each treatment plan consists of up to 9-12 hours per week of:
- Curated groups
- Individual therapy
- Family therapy
- Psychiatry, as needed
In addition to our standard treatment model, all clients have access to 24/7 crisis care, 20+ free family support groups, peer mentors, Alumni Programming, and more.
We design each treatment plan with one primary goal in mind: helping clients heal and rediscover hope. If you think you or a loved one could benefit from Charlie Health, please contact our Admissions Team to get started in as little as 24 hours.
How do I know if Charlie Health’s virtual IOP is right for me or my loved one?
Our mental health and substance use treatment programs, including our virtual Intensive Outpatient Program (IOP), are an appropriate level of care for those who need more than once-weekly therapy. Many of our clients have either recently discharged from a higher level of care (i.e. a residential treatment facility or emergency department admission) or are seeking more support than traditional weekly talk therapy.
While typical outpatient treatment involves one therapy session per week, our virtual IOP offers multiple hours of treatment per week. At Charlie Health, each client receives a customized treatment plan that includes three group sessions per week, one individual therapy session per week, and one family therapy session per week. We also offer psychiatric services as needed.
Different therapy modalities are used throughout sessions, creating a truly comprehensive treatment program.
How long is the program?
Our program typically lasts 9-12 weeks; your exact discharge date will depend on your overall treatment progress. Your Primary Therapist will help determine the best discharge date for your needs. View a sample schedule and learn more about how it works here!

Can I transition from detox or residential treatment to a virtual IOP?
Yes. Charlie Health is an ideal next step for individuals discharging from inpatient or detox programs who still need structured, ongoing support. Our virtual IOP provides rapid access to care—in as little as 24 hours after referral—and includes therapy, peer support, and optional medication management. This continuity of care helps maintain progress and reduces the risk of relapse during a critical stage of recovery.
Do you offer treatment options for people who aren’t ready to abstain from substance use entirely?
Yes. Charlie Health supports clients at every stage of their recovery journey. We offer both harm reduction and abstinence-based treatment options, so whether someone is evaluating their substance use or working toward complete sobriety, they can access care that aligns with their needs. This personalized approach fosters trust and encourages long-term engagement in treatment.
What is medication-assisted treatment (MAT), and how does it work at Charlie Health?
Medication-assisted treatment (MAT) is an evidence-based approach that combines FDA-approved medications—such as buprenorphine or naltrexone—with ongoing therapy to support individuals experiencing substance use disorders. At Charlie Health, MAT is offered in select states and fully integrated into our virtual IOP. This comprehensive approach supports both physical stabilization and emotional healing, helping each person move toward recovery on their own terms.
